RIN Quality Assurance Program Raytel Imaging Network’s (RIN) first and fundamental commitment is to provide access to conveniently located facilities of the highest quality. To participate in the network, facilities must meet and consistently maintain the level of excellence and quality service RIN demands of its network. To assure that only the highest-quality providers are participants in our network, RIN utilizes the ‘Quality Assurance Program.’ All RIN network facilities take part in this mandatory Quality Assurance Program. The ‘Quality Assurance Program’ is comprised of:
PROFESSIONAL/TECHNICAL REVIEW Quarterly, RIN randomly selects 5% of the total procedures referred to each facility for review of technical quality and radiological interpretation. RIN requests copies of specific patient films including patients’ history and radiological interpretation. Dr. Joshua Barnett, MD, RIN’s Medical Advisor, over reads the studies to evaluate the technical quality. The findings are shared with the facility and if appropriate, Dr. Barnett discusses with the facility’s Medical Director.
PROFESSIONAL CREDENTIALING
- All facility radiologists are required to complete a Physician Application in its entirety and submit with all credentialing documents. Primary and/or Secondary Source verification is done on all credentials submitted.
- Current CV
- Hospital Affiliations (if applicable)
- Professional Education/Post Graduate training/Continuing Education (if applicable)
- Current State Medical License – written documentation verified through respective State
- Physician Specialty Board Certification – written documentation verified through the Certifying Board (i.e. ABMS/AOA)
- Medicare/Medicaid Certification - verified through Office of Inspector General (OIG)
- Malpractice claim history - verified through National Practitioner Data Bank ( NPDB)
- DEA Certification –written documentation verified through DEA Database
- CDS Certification (if applicable) – verified through written documentation
- Current Malpractice Insurance Coverage – verified through written documentation
- RIN requires all Network facility technologists to provide the following documentation:
- Documentation of ARRT Membership/State License
- Copy of Continuing Education Credits
- CPR Training Certification
- TB Inoculation Certification
- MRI Accreditation (if applicable)
- Mammography Registration/Accreditation (if applicable)
- All Network facilities are required to complete a Facility Application in its entirety, submit a complete equipment summary and the following applicable documentation. Primary and/or Secondary Source verification is done on all documents submitted.
- Copy of required facility Licensure/Certifications – written documentation verified through respective State
- Ambulatory Care License (if applicable) – written documentation verified through respective State
- Commercial General Liability Insurance – verified through written documentation
- MRI ACR Accreditation – written documentation verified through American College of Radiology
- CT ACR Accreditation – written documentation verified through American College of Radiology
- PET ACR Accreditation – written documentation verified through American College of Radiology
- Mammography ACR Accreditation – written documentation verified through American College of Radiology
- Mammography FDA Certification – written documentation verified through US Food and Drug Administration
- Ultrasound ACR Accreditation – written documentation verified through American College of Radiology
- W-9 – Request for Taxpayer Identification Number and Certification
- RIN may request all Network facilities to provide the following documentation(s):
- Possession of Crash Cart/emergency kit
- Quenching Protocol (MRI only)
- Emergency/Code Call procedures
- Copy of yearly system service contract
- Ongoing preventive maintenance
- Cryogen contract (MRI only)
- Facility’s on-going quality assurance meeting minutes
CREDENTIALING OVERVIEW - Credentialing Policies and Procedures in place and updated ongoing.
- Credentialing Committee comprised of clinical peers, review and recommend network participation of all new facility and physician applicants.
- The credentialing process from the date of the completed application to the date of final credentialing decision shall not exceed 180 days. If the timeline is exceeded, the physician applicant will be required to re-sign and date the Attestation Statement and submit current documents to support the application.
- Re-credentialing of RIN participating facilities and physicians is completed every three (3) years.
- It is the sole responsibility of the facility and physician to forward all current documents as they expire and new documents are received.
- All Network providers are required to contact RIN immediately should there be any change in facility information (i.e. name change; address change; facility ownership; TIN change; changes to modalities). RIN must be contacted immediately upon addition of new physicians and/or removal of physicians from staff. All new physicians must complete a Physician Application and submit all required credentialing documents before they can be presented to RIN’s Credentialing Committee.
ADMINISTRATIVE QUALITY MANAGEMENT - RIN performs comprehensive site visits to Network facilities.
- RIN randomly mails a patient questionnaire to patients scheduled by RIN. The questionnaires are reviewed and if necessary, the facility will be contacted by RIN to provide positive, as well as negative comments, for discussion and review, in order to improve the quality of patient care.
